MUSCLE STRAIN

Muscle strain, also known as muscle tear or pulled muscle, happens when a muscle is stretched beyond its normal range by sudden pressure.

A strain of the muscle or muscle tendon is equal to a ligament sprain. A muscle strain occurs when muscle fibers cannot cope with the demands placed on them by exercise overload which leads to the tearing of the fibers.

MUSCLE INJURY CLASSIFICATION

Muscle lesions are classified as grade I, II, and III based on the number of fibers disrupted according to clinical and imaging investigations.

Grade I:  Mild Tear, <5%

  • Minimal Swelling
  • pain 
  • Tenderness
  • Minimal/ no loss of function or strength
  • Recovery time 7-21 days

 

Grade II: Partial tear 50%

  • Significant Pain associated with Swelling
  • Significant decrease in strength and function.
  • Recovery time is 2-3 months.

 

Grade III: Complete tear 100%

  • Severe Swelling
  • Severe Pain
  • Complete Loss of Strength and function
  • Recovery time six months or more than six months
  • Surgery might be required.

Treatment of Acute Strain : 

Follow PEACE PROTOCOL : 

P= Protect

  • Unload or restrict movement for 1 - 3 days
  • Minimise rest because Prolonged rest compromises tissue strength and quality.
  • Let pain guide the removal of protection and gradual reloading

E= Elevate

  • Elevate the injured limb higher than the heart as this promotes interstitial fluid flow out of the injured tissue.

A= Avoid Non-Inflammatory Modalities and avoid icing

  • Making use of Non-Inflammatory medications to inhibit the inflammatory process could impair the healing process.
  • Ice may disrupt inflammation, angiogenesis, and revascularization.
  • Ice may potentially delay neutrophil and macrophage infiltration
  • Ice may increase immature myofibers, which can result in impaired tissue regeneration and redundant collagen synthesis.

C= Compress

  • External mechanical compressions, such as taping or bandages, may limit edema and tissue hemorrhage, but they should still allow the full range of movement at the joint.

E= Educate

  • Patients need better education on their condition. It is critical for doctors to educate their patients and set realistic expectations about recovery times and optimal loading.
  • Load management will avoid over-treatment of an injury.

 

Follow LOVE protocol for the Sub-Acute stage a few initial days have passed.

L= Load

  • Patients with musculoskeletal disorders benefit from an active approach to movement and exercise.
  • Optimal loading without increasing pain promotes repair and remodeling, as well as building tissue tolerance and capacity of tendons, muscles, and ligaments.

 

O= Optimism

  • Stay realistic, but encourage optimism to improve the chances of an optimal recovery.
  • Positive thoughts in patient highly influence their recovery and prognosis so try to stay positive and realistic.

Barriers to recovery include psychological factors such as:

  • Catastrophising
  • Depression
  • Fear

 

V= Vascularisation

  • Musculoskeletal injury management needs to include cardiovascular physical activity, but more research is needed on the specific dosage. Pain-free cardiovascular activity is a motivation booster and increases blood flow to injured structures.
  • Early mobilization and aerobic exercise have the benefits of improving function, reducing dependency on pain medication, and facilitating recovery quickly.

E= Exercise

Classification is based on assessing the symptomatic response during repeated or sustained movement testing . Most patients' symptoms are classified into derangement syndrome, dysfunction syndrome, postural syndrome, or other conditions.

The choice of exercises in MDT is based on the direction that causes the symptoms to decrease or abolish.
