Disc Sequestration is the third type of IVDH which is characterized by the evacuation of a fragment of the disc nucleus and migration of this fragment from the disc space to another location and produces intense pain as well as neurological signs. Who Gets Disc Sequestration? 

Disc sequestration is common in people of working age between 30 and 50 years and doesn’t spare either sex but is more common in men due to high physical stress. From studies it has been estimated that between 5% to 10% of patients with lumbar disc herniation experience sequestration. It is found in such conditions like, lifting heavy weights in the workplace, little or no physical activities, and age related disc degradation.

Causes

  • Age-related Degeneration: Unfortunately, as people age the intervertebral discs get dehydrated and the elasticity decreases; therefore, they become prone to rupture and fragmentation. 
  • Heavy Lifting or Sudden Movements: The disc material can tear away due to excessive pressure on the spine or any sudden twisting and similar movement that puts pressure to the spine. 
  • Trauma or Injury: Injuries, falls, stokes or any other force that come on the spine and affect the discs may result in sequestration particularly if the force is very strong. 
  • Repetitive Strain: Any job or activity, which necessitates bending, twisting or constant lifting, exerts pressure on the discs which are likely to herniate and sequester. 

Sedentary Lifestyle: A relatively sedentary lifestyle aggravated by lengthy hours spent in front of a computer, can cause spinal muscles to become weaker holding the discs which then bulge through the annulus fibrosus.

Symptoms of Disc Sequestration

  • Severe Lower Back Pain: A sharp and severe pain that locals at the posterior aspect of the hip and often travels down the buttocks, thighs and legs and in most cases, it gets worse with movement or when sitting for long duration. 
  • Nerve Compression Symptoms: Sensory changes in leg and foot including numbness, tingling or weakness that is attributed to nerve root compression by the free fragment of the disc. 
  • Reduced Mobility: Pain often hinders the ability of patients to bend, walk or stand in patients with joint disorders thus narrowing down their daily activities. 
  • Sciatica: Intense, sudden pain that starts from the lower back region and shooting downwards the affected limb to lumbar disc sequestration claudication, that affects the sciatic nerve. 

Treatment

Diagnosis

  • Imaging Studies: MRI procedures are reckoned to be most effective in diagnosing disc sequestration as they accurately describe the structure of spine and location of free fragments of disc. 
  •  Functional Assessment: Measuring the patient’s ability to move around and maneuver within his or her environment, this usually involves comparing their movements before the onset of pain with pain induced restrictions.
