Ankylosing spondylitis is arthritis that causes inflammation in the joints and ligaments of the spine. It may also affect peripheral joints like the knees, ankles, and hips. Normally, the joints and ligaments in the spine help us move and bend. If you have ankylosing spondylitis, the inflammation in the joints and tissues of the spine can cause stiffness. In severe cases, this may cause the vertebrae (bones in the spine) to fuse (grow together). When the vertebrae fuse, it can lead to a rigid and inflexible spine.


Many people with ankylosing spondylitis have mild episodes of back pain and stiffness that come and go. However, others have severe, ongoing pain with loss of flexibility in the spine. In addition, other symptoms may develop depending on which other areas of the body are affected by the disease. Some people with ankylosing spondylitis develop eye disease (uveitis), skin disease (psoriasis), or gut disease (inflammatory bowel disease).


There is no cure for ankylosing spondylitis, but many treatment options help control symptoms. Recommended therapies may include exercise, physical and/or occupational therapy to improve mobility and posture, and medications to help manage pain, control inflammation, improve posture and body position, and slow the progression of the disease. With treatment, most people with ankylosing spondylitis can have productive lives.

Prevalence of AS in the population increases to approximately 5% among patients who are HLA-B27 positive.  AS occurs more frequently in men than women (2:1).  Age of disease onset usually peaks in the second and third decades of life.  Approximately 80% of patients with AS experience symptoms at ≤ 30 years of age, while only 5% will present with symptoms at ≥ 45 years of age.

Researchers do not know the cause of ankylosing spondylitis. However, studies show that both genes and environment may lead to the development of the disease. Researchers know that the HLA-B27 gene increases the risk of developing ankylosing spondylitis, but this does not mean one will get the disease if they have the gene. Many people have the gene and never develop ankylosing spondylitis, which tells researchers that environmental factors also play a role.  

Inflammatory back pain due to sacroiliitis and axial arthritis of the spine is frequently accompanied by peripheral arthritis, enthesitis, and/or acute anterior uveitis. Cardiac involvement resulting in aortitis and arrhythmias occurs less commonly.

  • Spinal inflammation results in symptoms of back stiffness, soreness, and pain. 
  • Symptoms tend to worsen in the early morning, causing sleep disturbance in many patients.
  • Pain, stiffness, and inflammation in other joints, such as the ribs, shoulders, knees, or feet.
  • Difficulty taking deep breaths if the joints connecting the ribs are affected.
  • Fatigue, or feeling very tired.
  • Abdominal pain and loose bowel movements.

Skin rashes, in particular psoriasis.

 

Who Gets Ankylosing Spondylitis?

Anyone can get ankylosing spondylitis; however, certain factors may increase your risk for developing the disease. These factors include:

Family history and genetics. People with a family history of ankylosing spondylitis are more likely to develop the disease.

Age. Most people develop symptoms of ankylosing spondylitis before age 45.   However, some people develop the disease when they are children or teens.

Other conditions. People who have Crohn’s disease, ulcerative colitis, or psoriasis may be more likely to develop the disease.

Effective treatment modalities will vary depending on whether symptoms include axial arthritis, peripheral arthritis, or enthesitis. All patients with AS should undergo vigorous physiotherapy to improve spinal mobility and physical functioning. Non-steroidal anti-inflammatory drugs (NSAIDs) are nearly always used in conjunction with physical therapy to alleviate symptoms of inflammatory back pain.

PROGNOSIS

The prognosis for many patients with AS will be complete spinal ankylosis, while others will experience intermittent flares between bouts of clinical remission.  The long-term goal in the treatment of AS is to preserve as much physical functioning as possible, decrease work absenteeism, and maintain a high quality of life.
